SBA is seeking volunteer “victims” to participate in their triennial full-scale emergency drill.

Moulage, food, and drinks will be provided.

Volunteers must be 18 years or older.

PHOTO RELEASE
I grant permission to the City of Santa Barbara and its agents or employees, to use photographs taken of me for use in any media publications on behalf or for the benefit of the City, including but not limited to recruiting brochures, newsletters, and magazines, and to use the photographs on display boards, and to use such photographs in electronic versions of the same publications or on City of Santa Barbara OES web site or other electronic forms or media, and to offer them for use or distribution in other publications, electronic or otherwise, without notifying me.
I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.

I am 18 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.

Statement of Hold Harmless and Understanding

Applicant agrees to investigate, defend and hold harmless the City, its officers, employees and agents from and against any and all loss, damage, liability, claims, demands, detriments, costs, charges, expense (including attorney’s fees) and causes of action of any character which the City may incur, sustain or be subjected to on account of loss or damage to property or the loss of use thereof and for bodily injury to or death of any persons (including but not limited to the property, employees subcontractors, agents and invitees of each party hereto) arising out of or in any way connected to the work to be performed pursuant to this agreement or occupancy, operation, maintenance, enjoyment and use of any City premises under this agreement to the extent permitted by law.
